Below is the 30-day public comment draft version of the CVEMSA Hospital Bypass policy 7008.

This policy replaces the published Trauma Bypass policy 7008. The new policy includes the content of the current 7008 but adds STEMI and Stroke bypass criteria as well. The policy also includes the criteria for closing an ED to incoming ambulance traffic in the event of a physical plant failure.

Changes to communication terminology are incorporated into the policy. Currently system partners are advised when an ED cannot receive suspected stroke patients due to CT scan unavailability by a “CT Scan Down” system notification. This notification will change to a specific “Stroke Bypass” advisory. STEMI Center unavailability is also clarified as “STEMI Bypass” within the policy.

Because the policy replaces rather than modifies the current Trauma Bypass 7007, The existing 7007 is included for reference.

Effective March 5, 2015

1. Revised Field Treatment Guidelines:

a) 8003 BLS Oxygen/Airway
Revised to reflect current evidence that too much supplemental oxygen can be harmful in certain conditions and outlines the use of titrated oxygen therapy.

b) 8004 BLS Pulse Oximetry
Revised to allow Pulse Oximetry along with assessment and other skills to be used as a BLS diagnostic tool to help guide oxygen titration.

This change affects the following treatment guidelines that refer to oxygen administration and have been revised to state:

"Consider oxygen administration per Treatment Guideline 8003."

2. New Field Treatment Guideline:

a) 8018 Hemostatic Agents - Optional Scope
The use of hemostatic agents is now an approved optional BLS skill. The use of any hemostatic gauze requires additional training provided by provider agency prior to use. The California EMS Authority has approved only Quick Clot® and Celox® dressings for use in the prehospital setting.

This change affects Treatment Guideline 8008 Trauma Management which has been revised to state:

"Use of gauze type hemostatic agents are an approved BLS Optional Scope to be used during tourniquet placement and in combination with direct pressure to aid in severe arterial bleeding or to control severe bleeding when tourniquets are not indicated (junctional injuries)."
