Report an EMS Event
Reporting is encouraged by all who encounter an actual or "potential" patient care safety event or recognize exemplary care in the field. These events may be related to systems, operations, devices, equipment, medication or any aspect of patient care and include "great catches" defined as patient safety events that are recognized and prevented before they actually occur.

Instructions for details communication:
- Provide a concise description of the event including date, time, location, and parties/agencies involved.
- Include your contact information: name, email address, and phone number.
- Reports submitted by provider agency representatives should include a brief summary of findings and disposition of the event.
Elder or Child Abuse Reporting
EMS providers are mandatory reporters of dependent elder and/or suspected child abuse. Suspected abuse must be reported in accordance with county-specific protection agencies rather than via CVEMSA event reporting.
Sonoma County:
Suspected child abuse reports are made with Sonoma County Child Protective Services. The Hotline and Reporting web page has links to the correct form and instructions on submissions.
Suspected dependent elder abuse reports are made with Sonoma County Adult Protective Services. The APS website has reporting information and resources for providers.
Mendocino County:
Suspected child abuse reports are made with Mendocino County Family and Children’s Services. The Social Services Child Abuse/Neglect reporting web page has instructions for report submission.
Suspected dependent elder abuse reports are made with the Mendocino County Department of Social Services, Adult Protective Services. Information on reporting is on the APS web page.
